Community Navigator Program (CNP)
The purpose of the Community Navigator Program is to help strengthen our community. Everyone has something to offer.
If you need assistance—
Need help finding housing, food or help with rent or heat? Want to return to work or find a better job? Worried about a loved one who is aging and not sure what resources are available to them or you? The Community Navigator Program (CNP) is a free and confidential service designed to help families on the Peninsula meet essential needs and achieve goals and aspirations. We connect community members to available resources so that everyone on the Peninsula can meet the challenges of life head on with grace and dignity.
The CNP serves as a single point of contact. Don’t know which way to turn or where to go first? Talk through your options with the Community Navigator.
What does the CNP do?
The Community Navigator assists members of our communities who are in need of a little help. We provide service coordination, technical assistance, financial aid (small grants) and resource coordination.
We provide referrals for:
Food assistance
Mental health and addiction services
Career counseling and entrepreneurship training
Health insurance including: Maine Care, Free Care, Market Place, and Medicare
Housing counseling & foreclosure prevention
Volunteer service opportunities
We also assist with:
Family budgeting and financial coaching
Small grants for basic living expenses (e.g., food, housing, fuel and utilities; self-care, including smoking cessation and substance abuse) to help members of our community to reach their potential
Communicating with your case manager, healthcare provider or other agency to help you make changes to your life
How it works:
Schedule an appointment (M-F, 9:30am-3:30pm) by calling (207) 350-1743 or email the Community Navigator, Hannah Corkum at hannah.corkum@crcboothbay.org
Your appointment will either be in person or by phone depending on your needs. We will make a plan and determine the action steps.
If you require financial assistance, please bring proof of income, bank or credit card statements statements, latest tax return, or some other official documentation of income and/or expenses.
About the CNP Community Navigator:
Hannah Corkum (LSW-C) is thrilled to be entering into her third year as Community Navigator for CRC. Hannah is a graduate of the University of Maine at Augusta, and has over ten tears experience working in the behavioral health field.
